Care beyond your visit

Plan how care will continue after you return home, through local clinical cooperation and the online follow-up arrangements agreed for your case.

A plan with your local healthcare team

Where appropriate, NEXA can work with a healthcare institution near you to coordinate an agreed follow-up management plan. This may bring together record handover, reviews, rehabilitation or ongoing monitoring. The participating team, responsibilities and actual services must be confirmed for your case.

Online follow-up through Care

A patient account in Care can support follow-up enquiries and communication about your agreed arrangements. Access to individual information depends on your relationship to the case and the permissions assigned to you. The responsible team must confirm the scope of clinical advice, specialist involvement and response arrangements.

Agree the handover before you leave

Confirm what records and reports you will receive, who coordinates follow-up, which reviews are planned, and how to raise questions after returning home. Online follow-up is not an emergency service; urgent concerns should be addressed through appropriate local medical care.

For local healthcare institutions

Discuss a continuing-care pathway with NEXA Care. Cooperation can define the clinical scope, handover, patient consent, responsibilities and escalation arrangements before a service is offered. A public institution profile does not by itself establish a cooperation agreement.
